This PR upgrades the `react-shadcn` template:
- Upgrades to the new Tailwind v4 styles and components
- Updates the example components to use the new ones.
- Removed unused form component
- Fixed some a11y issues with the example component.
### How did you verify your code works?
- Ran `bun build` to test if the template builds with no errors.

Fixes a duplicate output issue in `bun init` where `CLAUDE.md` was being
listed twice in the file creation summary.

cc @paperdave. Signals can be run from any thread. This causes an assertion failure when the receiving thread happens to not be the main thread. Easiest to reproduce on linux when you spawn 100 short-lived processes at once.
